Annual Information Tax Documents
Form W2: Form W2 is issued to all VCU employees who are US citizens, resident aliens and non-resident aliens without a treaty who have had taxable federal, social security, medicare and state wages paid during the calendar year. The form is released on or before January 31st of the year following the year the compensation was received. If January 31st falls on a weekend, the forms are issued no later than the next business day.
Form 1042: Forms 1042 is issued to nonresident aliens who have worked during the calendar year and received wages that were exempt from federal income tax due to a treaty between their home country and the United States. Non-employed athletes or students who are the recipients of non-qualified scholarships also receive a Form 1042. The form is released on or before March 15th of each year following the year the money was received.
Form 1099R: Form 1099R is issued by the VCU Payroll department to all participants in VCU FERIP (Faculty Early Retirement Incentive Program) that have received any distributions during the calendar year. The form is released on or before January 31st of the year following the year the compensation was received. If January 31st falls on a weekend, the forms are issued no later than the next business day.
Current Tax Forms
Federal Income Tax Form W4: Can be completed by visiting employee self service or submitting the paper W4 and submitting it to VCU Payroll services.
Virginia State Income Tax: VA-4: Can be completed by submitting the paper VA-4 to VCU Payroll services.California State Income Tax: DE-4: Can be completed by submitting the paper DE-4 to VCU Payroll services.

Form SS-5 Application for a Social Security Card
Used to obtain a new card or a replacement card.
Form 8233 Exemption From Withholding on Compensation for Independent (and Certain Dependent) Personal Services of a Nonresident Alien Individual
Used to claim exemption from withholding on compensation earned for personal services due to an income tax treaty or personal exemption amount.
Form W-7 Application for IRS Individual Taxpayer Identification Number
This form is used to apply for an individual taxpayer identification number. It is used by persons who are not citizens and ineligible to apply for a Social Security number. The ITIN is used to file a Federal tax return of US sourced income received.
Form W8-BEN Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding and Reporting (Individuals Only)
- Establishes foreign status
- Claims the individual is the beneficial owner of the income
- Allows the individual to claim a reduced rate or exemption regarding withholding under a tax treaty.
Form W-9 Request for Taxpayer Identification Number (TIN) and Certification
Used by a payee that is a United States person but not an employee.
- Provides the payee’s tax identification number (TIN) for reporting purposes required by the payer. The recipient will receive a Form 1099 at year end.
